The 10 Most Commonly Notarized Legal Forms

From bills of sale to name changes, here are the legal forms that bring people to the notary most often — and what each one requires.

The most-notarized forms in any given week: vehicle bill of sale, vehicle title transfer, general affidavit, statutory power of attorney, advance healthcare directive, residential lease addendum, child travel consent, sworn statement of identity, name change affidavit, and small estate affidavit.

Most are short and require only an acknowledgment or jurat. The signer brings the unsigned form, a photo ID, and (for jurats) prepares to swear to the document's truth.

State-specific variations are common. A small estate affidavit good in Texas isn't valid in California. When in doubt, use your state's statutory form — most Secretary of State websites publish current templates.
